This form is used to provide access for our external partners to Ministry Applications such as HAMIS, Hospital Out of Province Claims (HOOPC) or Medical Imaging Database (MID).

Your Access Administrator/Supervisor will receive an electronic copy of this request and MUST APPROVE and FORWARD this request to ITSB.AccessTeam@gov.bc.ca.
